PARTICIPANT/GUARDIAN ACKNOWLEDGEMENT AND RELEASE
By signing this waiver, I, the undersigned participant and/or guardian, acknowledge that martial arts and yoga activities involve inherent risks of physical harm, including but not limited to physical exertion, injury, and even death. I voluntarily assume all risks associated with participation in activities at Scratch
Line Enterprise LLC.
I agree to hold harmless and blameless all individuals in any way associated with Scratch Line Enterprise LLC, including but not limited to its owners, directors, instructors, employees, independent contractors,
and affiliates. I understand that it is my responsibility to inspect the premises prior to participation and determine to my own satisfaction that they are safe. If I have any pre-existing physical conditions that
may be aggravated or re-injured, I confirm that I have taken all necessary precautions and will refrain from participating in activities that could cause injury.
ASSUMPTION OF RISK AND WAIVER OF LIABILITY
I acknowledge the possibility of serious injury during training and other activities at Scratch Line Enterprise LLC. I hereby assume all risks of injury and damages, however and wherever they may occur, both for myself and any minor for whom I am the legal guardian. I release Scratch Line Enterprise LLC and all its associated parties from any liability for injuries or damages should they occur.
SEXUAL MISCONDUCT POLICY
Participants are strictly prohibited from engaging in any form of sexual molestation or misconduct. This includes, but is not limited to, unwanted physical contact, verbal harassment, or any other behavior of a sexual nature. Any occurrence of such misconduct will result in immediate expulsion and/or termination of the contract and may lead to legal action. No refunds will be issued in such cases.
RECORDING POLICY
Visual or audio recording of any participant is strictly prohibited without written consent from Scratch Line Enterprise LLC.
PHOTO RELEASE ACKNOWLEDGEMENT
I hereby grant Scratch Line Enterprise LLC and its representatives the irrevocable and unrestricted right to use and publish photographs and/or video recordings taken of me for purposes such as marketing, promotion, or social media. I acknowledge that these images may be used in various publications, websites, social media platforms, or other promotional materials related to Scratch Line Enterprise LLC. I waive any right to inspect or approve the final product in which my likeness appears and acknowledge that I will not receive any financial compensation. I release and discharge Scratch Line Enterprise LLC
and its representatives from any claims, demands, or causes of action related to the use of these images.
